About this Site

The Vermont Transition Guide for young adults with Autism Spectrum Disorders and Developmental Disabilities was developed in response to a need for best practice guidelines and accessible information related to the transition planning process. This website was fully funded by a grant to the Vermont Department of Health from the United States Department of Health and Human Services, Health Resources, and Services Administration. While the project was coordinated by HowardCenter, it’s the result of collaborative efforts by numerous young adults, parents and professionals across the state. This site will continue to be maintained and updated with the collaboration of Vermont Family Network.

Although this transition guide was developed to specifically support successful transitions to adulthood for youth with Autism Spectrum Disorders and Developmental Disabilities, it could also be a valuable tool for other young adults preparing for life after high school.  All are welcome to use it as a transition resource.
